Jujutsu Kaisen chapter 257 on leaks opens with a flashback in which Sukuna discusses Yuji with Uraume. He queries Uraume about Yuji’s true identity as he possesses an incomprehensible level of strength, even from the cursed energy that remains after Sukuna leaves his body.
This flashback is from Sukuna’s pre-awakening period while he was still inside Megumi.
Sukuna discloses that in order to stay healthy while still in his mother’s womb, he ate his twin brother. Later on in the story, Kenjaku managed to capture his deceased brother’s spirit, and Yuji Itadori was born as a result.
Update: A translation error was made, based on the latest spoilers. Sukuna Itadori’s twin brother is not Yuji Itadori. The Twin’s spirit passed through a number of individuals after Sukuna swallowed it before ending up in Jin Itadori. Following Kenjaku’s mating with Jin Itadori, Yuji Itadori was born. In a technical sense, Yuji Itadori is Sukuna’s nephew, or descendant, if you like.
This makes Uraume question whether Yuji and Sukuna have similar intrinsic potential. The flashback is followed by the return of the series to Yuji and Sukuna’s fight. Unexpectedly, Yuji uses two Black Flashes in quick succession to attack Sukuna.
But after being pushed back, Yuji falls on a post and cuts it before throwing it at Sukuna. At the moment, Yuji is carrying two cursed techniques: Sukuna’s cursed technique, Mizushi shrine, and Blood Manipulation from Blood painting.

When Sukuna lands, he ruins the space they are in and tries to attack Yuji, but Yuji grabs hold of him and punches Sukuna back into the wall. Sukuna slashes at Yuji’s face when he unleashes another powerful Black Flash on him.
Yuji smacks Sukuna with yet another flashback despite the blood streaming from his face. Sukuna becomes even more upset at this point and screams at Yuji, but Ino throws Nanami’s blunt knife at him in a final attempt to stop him.
Yuji is fired up by Ino and has enough time to get ready for the ultimate Black Flash. Itadori smashes Sukuna with his eighth consecutive Black Flash at the end of the chapter.
